皮肤科的“战痘”日记:从油脂叛乱到科学镇压

Dermatology's battle against acne: from sebum rebellion to scientific suppression

摘要: 以皮肤科诊室为背景,梅医生与徒弟小窦的对话为主线,揭示痤疮(痘痘)的形成机制:雄激素刺激皮脂腺过度分泌油脂,与角质细胞混合堵塞毛囊形成闭口/黑头(氧化后呈脂褐素沉积),缺氧环境下痤疮丙酸杆菌分解油脂引发炎症,导致红肿脓疱。中医通过长痘部位判断内因——额眉鼻根对应肺热心火,口周面颊提示脾胃湿热,面颊下巴反映肝气不畅。治疗推荐水杨酸:其脂溶性可溶解角质疏通毛孔,抑制皮脂腺活性及致病菌繁殖,兼具抗炎和促进表皮更新功能。故事通过生理病理与生活干预结合,传递科学护肤理念。

Abstract: Set in a dermatology clinic, the dialogue between Dr. Mei and apprentice Xiao Dou elucidates acne pathogenesis: androgen-stimulated sebaceous overproduction combines with keratinocytes to obstruct follicles, forming comedones (oxidized to lipofuscin deposits). Under hypoxic conditions, Propionibacterium acnes metabolizes sebum, triggering inflammatory responses that manifest as erythema, edema, and pustules. Traditional Chinese Medicine diagnostics correlate facial acne distribution with internal imbalances: forehead/brow/nasal lesions indicate lung-heat and cardiac fire, perioral/cheek eruptions suggest spleen-stomach damp-heat, while mandibular acne reflects liver qi stagnation. Therapeutically, salicylic acid demonstrates multifunctional efficacy: its lipophilic nature enables keratin dissolution and pore unclogging, while suppressing sebaceous activity and microbial proliferation, coupled with anti-inflammatory and epidermal renewal properties. This narrative integrates pathophysiological mechanisms with lifestyle interventions to promote evidence-based skincare paradigms.
